Editorial 'From darkness into light': time to make awake intubation with videolaryngoscopy the primary technique for an anticipated difficult airway? In this month's issue of Anaesthesia , Kramer et al. present a study that advances current practice for awake intubation in a patient with an anticipated difficult airway [1]. Hitherto, this has usually been achieved using a fibreoptic 'scope, but the study describes an approach that could change this practice. Kramer et al. randomly assigned patients with an anticipated difficult intubation to either an awake intubation using a fibrescope, or laryngoscopy using a C-MAC â vid-eolaryngoscope with a D-BLADE.

Awake fiberoptic or awake video laryngoscopic tracheal intubation in patients with anticipated difficult airway management: a randomized clinical trial.

BACKGROUND Awake flexible fiberoptic intubation (FFI) is the gold standard for management of anticipated difficult tracheal intubation. The purpose of this study was to compare awake FFI to awake McGrath® video laryngoscope, (MVL), (Aircraft Medical, Edinburgh, Scotland, United Kingdom) intubation in patients with an anticipated difficult intubation. گزارش یک مورد بلوک راه هوایی در بیمار با توده حنجره و احتمال انتوباسیون مشکل

Local block and awake intubation are the choice for airway management in patients with anticipated difficult airways. This paper reports a case of a 67 year old man with snoring and hoarseness and laryngeal mass in stroboscopy (12X12X10), candidate for elective surgery. He was intubated awake and the mass was resected. Therefore, patients with horseness and snoring should not be anesthetized im...

Awake videolaryngoscopy-assisted tracheal intubation of the morbidly obese.

Awake videolaryngoscopy may be useful for the tracheal intubation of the morbidly obese. This prospective, observational study enrolled 50 patients undergoing bariatric surgery. After sedation and topical anaesthesia of the airway, awake tracheal intubation was attempted, assisted by videolaryngoscopy, and terminated if there was severe gagging, coughing, or inadequate laryngeal view. After thr...

Laryngeal Radiation Fibrosis: A Case of Failed Awake Flexible Fibreoptic Intubation

Awake fibreoptic intubation is accepted as the gold standard for intubation of patients with an anticipated difficult airway. Radiation fibrosis may cause difficulties during the intubation procedure. We present an unusual severe case of radiation induced changes to the larynx, with limited clinical symptoms, that caused failure of the fibreoptic intubation technique. A review of the known lite...
